Ctrl+Click the toolbar to customise it and it will allow you to add it to
the toolbar.

The problem is that if you had a version of QLab installed before the
Refresh Targets button existed then QLab's preferences file doesn't have an
entry saying you want it on the toolbar and therefore it therefore QLab
doesn't display it.

Perhaps if QLab detects that the .plist file was written by a previous
version of QLab it should offer to zap either the whole thing or selected
bits of it that are known to have changed and reset to defaults.
