[QLab] Only two outputs are enabled

Hey DT,

Good to hear from you, and thanks for the suggestion.  Yeah, it is a
G5 so I'll look into the driver issue sometime this week.

Qlab works beautifully with my MOTU 896 at home, so I was just sort of
surprised by this issue on the 2408. I guess I've been spoiled by my
time on a Mac (and Qlab) - where everything usually just works.  I'll
have to poke under the hood a bit more on this one.

Hope all is well in NYC.
