[QLab] Only two outputs are enabled

It looks like it is a MOTU 2408 problem.

Everything works fine using an M-Audio 410.

Chris, is there anything that can be done to get Qlab to recognize
more than 2 outputs on a 2408? (I've updated the 2408 to the latest
driver)
