Just a little update on the reported glitches:
I've fixed the "fails to reset the first frame" problem, and cleaned
up a few more rough spots that I noticed in the process.

I'll spare you the gritty details, but it turns out this *appears* to
be a bug on Apple's end. After poking at it for the last two days, I
eventually tried Apple's own sample code:

And lo and behold the same thing happens for the MPEG1 files I've
tried. I've posted a question about this to the Quicktime mailing
list, but no one ever responds to my queries on that list, so I don't
know if that will produce any answers.
With the new cleaned up cue I'm seeing other formats (AVI, MOV, etc.)
play through to the final frame properly, so it may end up that I'm
simply not able to support the MPEG1 format properly. Hopefully that
won't be too much of a bother.
