It's likely this is the same issue that Steve Devino first reported.   
I researched it carefully, and posed some questions to Apple's OpenGL  
mailing list.  It was determined that this can occur when the display  
device is not calibrated correctly (be it a monitor or projector).

To confirm this I used a Spyder2Express to calibrate the monitor  
where I was seeing the behavior:

This fixed the problem on my external monitor.  It appears their  
higher-end product also can be used on projectors, although I have  
not used it myself.  The cheaper option is to try to calibrate as  
carefully as possible by hand.
