Open up:

/Applications/Utilities/Audio MIDI Setup.app

Go to the menu item:

Audio -> Open Aggregate Device Editor

Create your aggregate device.

You'll need an Enhanced Audio License for QLab to recognize the  
device, but it will just show up as a normal device that you would  
use just like any other device.

One thing to be aware of is that aggregate devices require more  
computing power to use, so be cautious if using an old machine or an  
already CPU-hungry show.
