Hi Joshua,
Unfortunately, as far as I can tell, external MIDI devices are not
supported through QuickTime on OS X. (This was true at least for the
first few versions of OS X, and I have not been able to find whether
they fixed this or not in Tiger, and if so how to use it.) Quicktime
appears to only support substituting in alternate sound banks for use
on the local machine.
I'll shoot off a quick email to the Apple list and see if the
external devices are still unsupported. If so, it means I'll have to
create a MIDI Music Cue for playing back MIDI files to external devices.
