Another thumbs up for MidiStroke - it is a little restricted in so much as 
you can't save/load setups (it stores settings on exit, and recalls when 
loaded).  What it does do is enable you to trigger QLab using standard MIDI 
note ons, (or CCs) which means you can set up Q Lists to trigger from 
seperate Midi notes, or set up one-shot sound FX that could be triggered 
live by an MD's keyboard etc.
(until MIDI triggers become native to QLab hint hint!!! (I hope)).

Another small device you could use (for local operation)  would be the 
Contour Design Shuttle Express 
http://www.contourdesign.com/shuttlepro/shuttlexpress.htm , although the 
buttons are a little bit loud (click wise).  You can program the buttons to 
give you Go, Stop etc, and use the jog wheel to navigate up and down the Q 
List.
